About The DAISY Foundation
The DAISY Foundation was established in 2000 by the Barnes family in memory of their son, J. Patrick Barnes, who died from complications of Idiopathic Thrombocytopenic Purpura (ITP) at theÌýage of 33. The FoundationÌýis dedicated to funding ÌÇÐÄ´«Ã½ to help fight diseases of the immune system and supporting ITP patients and their families.Ìý Having been touched by the remarkable care, clinical skills, and compassion demonstrated by nurses during Patrick’s illness, the Barnes family made it their mission to recognize exceptional nurses around the country.

What is the DAISY Award?

Presented in the award recipient's unitÌýÌý
All recipients throughout the year are recognized during Nurses Week
Recipients will receive: aÌýframed certificate, DAISY Award signature lapel pin, hand-carved stone sculpture entitled A Healer’s Touch, and a copy of their nomination letter
The department of the recipient will receive Cinnabon cinnamon rolls – Patrick’s favorite treat during his illness
How to nominate an extraordinary nurse
Patients, visitors, employees, or physicians may nominate a deserving nurse.
Fill out the onlineÌýÌýor submit a paper form at any of the DAISY nominations boxes in patient care areas.
Criteria/attributes for nominations should include ÌÇÐÄ´«Ã½ Tri-Valley values and must includeÌýspecific examples of a clinical situation where the specific nurse contributed to our Mission and Vision by providing a compassionate patient care experience.

Nominations can be submitted all year long. Selection process takes place quarterly. Recipients will be notified by their department's manager.ÌýÌý

What is the selectionÌýprocess for the award?
Only nominations with aÌýpatient's story will go toÌýthe final selection process.
The selection process is blinded - each final nomination letter is coded for anonymity, to prevent bias amongÌýthe Selection Committee members.
Members of the DAISY Award Selection Committee will read all finalÌýnomination letters and grade them based on the presence of ÌÇÐÄ´«Ã½ Tri-Valley values and theÌýquality of the story.
A scoring grid is used to grade the nomination letters.
