Atrial fibrillation (AF) ablation by pulmonary vein isolation (PVI) is the cornerstone of therapy for symptomatic AF, improving quality of life and prolonging survival in several populations (1,2). Nevertheless, the long term success of PVI is still in the range of 40-70% in patients with paroxysmal and persistent AF (3). A better understanding of mechanisms of fibrillatory conduction, beyond targeting the triggers initiating AF; should contribute to the ultimate goal to improve ablation outcomes for AF patients.
