Continuing Care Network (CCN)
At ÌÇÐÄ´«Ã½, we want to help you get better even after you leave the hospital. That's why we created the Continuing Care Network (CCN).
The CCN is a group of health care providers. These providers have made commitments to ÌÇÐÄ´«Ã½ to help you receive the care you need to recover after a hospital stay. We select our CCN partners after reviewing CMS data. CMS is short for the Centers for Medicare & Medicaid Services.

Our CCN network includes:
- Skilled Nursing Facilities (SNF): They offer rehab and care you can't get at home.
- Home Health Agencies (HHA): Providers visit your home to help with your health needs.

The CCN is a network of providers that has agreed with ÌÇÐÄ´«Ã½ to meet certain quality and care coordination standards designed to more seamlessly coordinate your post-acute care. Each CCN provider is independent from ÌÇÐÄ´«Ã½, Stanford University, and their affiliates. ÌÇÐÄ´«Ã½, Stanford University, and their affiliates do not own or operate any of the CCN providers and do not control the care provided by the CCN providers.
