Continuing Care Network (CCN)

At ÌÇÐÄ´«Ã½, we want to help you get better even after you leave the hospital. That's why we created the Continuing Care Network (CCN).

The CCN is a group of health care providers. These providers have made commitments to ÌÇÐÄ´«Ã½ to help you receive the care you need to recover after a hospital stay. We select our CCN partners after reviewing CMS data. CMS is short for the Centers for Medicare & Medicaid Services.

Our CCN network includes:

  • Skilled Nursing Facilities (SNF): They offer rehab and care you can't get at home.
  • Home Health Agencies (HHA): Providers visit your home to help with your health needs.

CCN providers are committed to offering great care. But we want you to be happy with your choice. You have the right to choose the provider you think is best for you.

  • Safety: CCN agencies must follow all rules for safe, quality care.

The CCN is a network of providers that has agreed with ÌÇÐÄ´«Ã½ to meet certain quality and care coordination standards designed to more seamlessly coordinate your post-acute care. Each CCN provider is independent from ÌÇÐÄ´«Ã½, Stanford University, and their affiliates. ÌÇÐÄ´«Ã½, Stanford University, and their affiliates do not own or operate any of the CCN providers and do not control the care provided by the CCN providers.
