Extended Care Team
Your care coordinator is your dedicated clinic contact. In each visit, you will meet with both your primary care provider and care coordinator. This allows your provider to focus on you while your care coordinator takes notes. Your coordinator will also manage referrals and provide follow-up education. You can connect with your coordinator by phone or email to ask questions and coordinate quickly with your care team at large.
Our physical therapists offer multispecialty, individualized care with state-of-the-art technology and equipment. Our providers are highly-skilled, specialized therapists with advanced certifications and follow protocols developed with Stanford doctors.
Our clinical pharmacists collaborate with your doctor to address your health needs. For certain health care conditions, they can order necessary lab tests, start, adjust, or stop medications, and provide thorough education. Our dietitian provides nutrition screening, assessment, intervention, and dietary counseling for a wide variety of conditions such as diabetes, digestive health, and weight management.
We have a robust team for diabetes management including physicians, Advanced Practice Providers (APPs), pharmacists, diabetes educators, and a dietician. We also offer diabetes education classes.
Diabetes Care Program
